Introduction
In 2021, QYO is proceeding with two music camps:
Fri 5 to Sun 7 Feb - JSE Music Camp at QCCC Brookfield
Fri 19 to Sun 21 Mar - WE Music Camp at QCCC Tamborine

The decision has been made after taking into account a number of factors including:
- The Covid plan in place for each camp site, including room capacities for rehearsals and tutorials
- Smaller size of each ensemble and lower age of members compared to other QYO ensembles

General
We encourage all members of JSE and WE to attend their music camp. The camp starts late on Friday afternoon and ends on Sunday early afternoon. It comprises rehearsals, tutorials and some limited recreational activities. The cost is included in the membership fee for JSE and WE. Those unable to attend the camp will be eligible for a membership fee refund of $120. Bus transport is provided for WE but not for JSE.
Each ensemble’s Administrator will issue a camp booklet PDF with details about the itinerary, what to bring, campsite location and other information. The WE Administrator will also survey members to confirm numbers for the bus transport.

For Members who can attend, we require parents of JSE and WE members, and those WE members who are aged 18, to complete the form below.

1. For members currently aged under 18: I am a Parent/Guardian of the musician whose details I have provided in this Permission Form. I give permission for my child to attend the 2021 QYO Music Camp, including, but not limited to, bus transport (for WE camp only), rehearsals, sectional tutorials and recreational activities. Or, For members currently aged 18 or over: I will be attending the 2021 QYO Music Camp.

2. I agree to be bound by the QYO Performer Member Information, Terms and Conditions and other QYO Policies, being those existing as at the time of acceptance of me / my child as a QYO member, and thereafter as lawfully modified and amended or added to from time to time.

3. I hereby release and indemnify QYO, its officers, volunteers, agents and members against any liability in relation to my / my child's participation in QYO activities, and when traveling to and from such activities.

4. I authorise QYO, in the event of any injury or illness occurring during QYO activities, to obtain on my behalf and at my expense any medical treatment as may be considered appropriate by QYO.

5. I am aware of the QYO Privacy Policy available from the QYO website and I consent to the information collected through this form and through QYO activities to be used in accordance with the rights and obligations set out in that policy.
