For which orchestra are you accepting a position in 2021?

If you have been offered a Reserve position (on the waiting list) as well as an offer to join as a Performer Member, you need to respond to the Reserve offer separately by emailing auditions@qyo.org.au.

If you have been offered two positions in two orchestras, permission must be granted via the office from both conductors and then two membership forms must be submitted. QYS and QYO2 members are not allowed to join a second orchestra, except Big Band.

MEMBER/PARENT DECLARATION

1. For members currently aged under 18: I am a Parent/Guardian of the musician whose details I have provided in this Membership Form. I give permission for my child to participate in Queensland Youth Orchestras (QYO) activities in 2021, including, but not limited to, rehearsals, concerts, tutorials, workshops, day trips and tours. Or, For members currently aged 18 or over: I accept the QYO position offered to me in 2021 as detailed in this Membership Form.

2. I agree to be bound by the QYO Member Information, Terms and Conditions and other QYO Policies, being those existing as at the time of acceptance of me / my child as a QYO member, and thereafter as lawfully modified and amended or added to from time to time.

3. I hereby release and indemnify QYO, its officers, volunteers, agents and members against any liability in relation to my / my child's participation in QYO activities, and when traveling to and from such activities.

4. I authorise QYO, in the event of any injury or illness occurring during QYO activities, to obtain on my behalf and at my expense any medical treatment as may be considered appropriate by QYO.

5. I grant QYO the right to use my/my child’s name, likeness, voice and biographical information for QYO marketing and fundraising purposes and I grant QYO the exclusive license to record, reproduce, sell reproductions of, and broadcast any QYO performances in which I/my child is involved. This includes the audiovisual recording, production and distribution of the 2021 QYO Finale Concert DVD.
