1. I am a Parent/Guardian of the musician whose details I have provided in this Permission Form. I give permission for my child to participate in the John Curro National Youth Concerto Competition including, but not limited to, transport, rehearsals, workshops, recitals, recreational activities and billeted accommodation. Or, for members currently aged 18 or over: I will be attending the 2022 QYO3 Tour. *

2. I have read the NYCC rules. My Child agrees to be bound by the QYO Performer Member Information, Code of Conduct, Terms and Conditions and other QYO Policies, being those existing as at the time of acceptance of my child as a NYCC participant member, and thereafter as lawfully modified and amended or added to from time to time. *

3. I hereby release and indemnify QYO, its officers, volunteers, agents and members against any liability in relation to my child's participation in QYO activities, and when traveling to and from such activities. *

4. I authorise QYO, in the event of any injury or illness occurring during QYO activities, to obtain on my behalf and at my expense any medical treatment as may be considered appropriate by QYO. This includes headache tablets such as paracetamol as prescribed by the manufacturer. *

5. For QYO3 Tour Members aged under 18: If my child tests positive for COVID during the tour, I or another delegated adult authorised by me, agree to immediately travel to Bundaberg to bring my child back to Brisbane. For QYO3 Tour Members aged 18 or Over: If I test positive for COVID during the tour, I will immediately leave the tour and adhere to Queensland Government regulations regarding isolation. *

6. I am aware of the QYO Privacy Policy available from the QYO website and I consent to the information collected through this form and through QYO activities to be used in accordance with the rights and obligations set out in that policy. *

7. I hereby grant QYO permission to lawfully use, edit, alter, copy, exhibit, publish, or distribute my Child’s likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration in perpetuity *
