All QYS members are asked to submit this form by 30 June regarding their instrument(s) in its travel case, exactly as it will be packed for tour, including any accessories. Percussionists only need to provide details for their stick/mallet bags. Please measure your instrument case at its widest points, eg. for those with moulded cases.
